One of the Most Effective Treatments in Psychiatry
Electroconvulsive Therapy (ECT) is a highly effective, evidence-based treatment for severe depression and certain other psychiatric conditions. Modern ECT is very different from the outdated portrayals many people have seen in movies or television. Treatment is performed under general anesthesia with continuous medical monitoring, allowing ECT to be delivered in a controlled and carefully supervised setting.
At the Alabama Institute of Mood and Memory (AiMM), ECT is part of our comprehensive Interventional Psychiatry Program, alongside Transcranial Magnetic Stimulation (TMS) and SPRAVATO® (esketamine).
ECT treatments for AiMM patients are performed at Huntsville Hospital, where patients receive anesthesia and appropriate medical monitoring throughout the procedure.
When Is ECT Considered?
ECT may be recommended when a psychiatric illness is severe, urgent, or has not responded adequately to other treatments. At AiMM, we carefully evaluate available treatment options and generally consider ECT when clinically appropriate after less-invasive treatment approaches have been explored, unless the severity or urgency of the illness makes ECT the preferred treatment.
ECT may be particularly helpful for:
Severe or treatment-resistant depression
Depression associated with psychotic symptoms
Severe depression with significant suicidal thoughts or behaviors
Catatonia
Certain severe episodes of bipolar disorder
Selected cases of severe psychiatric illness when medications or other treatments have not been effective or cannot be safely tolerated
In some situations—particularly severe depression, catatonia, or illness requiring a rapid clinical response—ECT may be considered earlier because of its established effectiveness.
How Does ECT Work?
During ECT, the patient receives general anesthesia and a muscle-relaxing medication. Carefully controlled electrical stimulation is then used to produce a brief therapeutic seizure.
The procedure itself takes only a few minutes, although additional time is required for anesthesia preparation and recovery.
Patients are closely monitored before, during, and after treatment. Because anesthesia is used, patients need someone to drive them home following outpatient treatment and must follow the specific instructions provided by their treatment team.
How Effective Is ECT for Depression?
ECT is considered one of the most effective treatments available for severe major depression, including depression that has not responded to multiple medications or other therapies.
Some patients begin to experience improvement relatively early in a treatment course. The number and frequency of treatments are individualized according to the patient's condition, response, tolerability, and overall treatment plan.
As with any medical treatment, individual results vary.
What About Memory Loss?
Memory concerns are among the most common questions patients and families have about ECT.
Temporary confusion and difficulty remembering events around the period of treatment can occur. Some patients experience gaps in memory for events occurring before or during the ECT treatment course. Cognitive effects vary considerably among individuals and may be influenced by the type of ECT used, electrode placement, treatment parameters, number of treatments, age, and other medical factors.
